FOUR DISTRICT TEAM LIST (.doc)	This is a team list that is given to the Referee and Opposing Team before every game.
BDYSA Change to Team Staff Form (.doc)	This form is submitted to your Club Registrar (who forwards it to the District Registrar) whenever you add a team official (Coach, Assistant, Trainer, Manager) after the team list is submitted at the start of the season.
BDYSA INDIVIDUAL PLAYER REGISTRATION Form (.doc)	This form is used to add a player to your team list after the list has been completed and  submitted to the Club  Registrar.
BDYSA INDIVIDUAL PLAYER Withdrawal Form (.doc)	This form is used to remove a player from your team list after the list has been completed and  submitted to the Club  Registrar.
BcSA Mini team registration Form (.pdf)	This form is used to register you Mini age group team with BC Soccer at the start of the season. When completed it is submitted to the Club  Registrar.
BcSA APPLICATION TO PLAY OUT OF DISTRICT Form (.pdf)	This form is required for any players on your team who live outside of the City of Burnaby. They will not be registered by the District without this form.
BcSA APPLICATION TO TRANSFER - YOUTH Form (.pdf)	This form must be completed whenever a Player registered with a team wishes to transfer to another team. There is a $10 fee for this action.
BcSA RISK MANAGEMENT Form (.pdf)	A Risk Management Form must be filled out and filed for every person over the age of 19 years who is left in charge of, or in regular contact with, any youth age player.
